Ring light makes her eyes look as if there is something wrong!

Are the ring lights in the center of the eyes created from the camera lens or are they added with software with post processing or something else entirely? Most portraits I see don't have these white rings in the eyes. But some do, which is what leads me to believe that they are added in post processing. But maybe they are a result of how the light is captured with the lens.
