Afternoon around about 2 o'clockLighting
This was a one light setup used from the side, with a beauty dish attachment. The dark studio helped a lot with controlling the light and having strong contrast from light to dark. Having a honeycomb on the beauty dish helped for more control over the light and where to direct it.Equipment

Yes I used photoshop just for some retouching, this is mainly just to smoothen out the skin and remove any unwanted marks on her face.In my camera bag

To capture this images you need a dark studio or environment where you can control the light and concentrate it where you want. A studio lens is needed to ensure her feature are all in proportion and that she doesn't have bigger nose than usual. Be sure that you don't lose any detail thanks to shadows falling over her face. And lastly always find something to make the image a bit different than just another portrait, for example adding a hand into the image.
