Use a model with experience in underwater photography, test her out in advance, be prepared for unexpected things (have someone keep an eye on her, while and between you are taking photographs, certainly in a deep pool or pond ALWAYS in open water). DO NOT TAKE UNEXPERIENCED MODELS IN OPEN WATER !! This is a recipe for failure and possible injury or drowning. Do breathing exercises in advance of shooting in the water and staying under with lead and without. Take it easy, take your time and have fun. Control your own breathing and when in open water, lock your camera to your body with a quicklock. Use the right equipment, ALWAYS have extra hands on deck for safety.
