Thank you for your interest Jacobundy! I created this picture of the trees in the park by stitching about 180 single photo's together to form this polar panoramic view. I've used a free stitching programme called Image composite editor. In order to create the 360 degree final image the challenge is to get enough overlap between each picture taken. I used a tripod for more accurate result but it's not necessary.

So, here is my guess on how this was setup, and because you already spelled out some of it :D.
Low tripod position placing the horizon about 1/3 the way up the view (what 6" off the ground?) and focused to infinity. Then rotating about 2 degrees per shot, when you merged the shots together you got a nice long panorama, and were able to use either a fisheye or polar transform to assemble.
