This shot was set as a monochrome in the camera's profiles, as I believe that if your intention is to shoot B&W, then you should set your camera in that mode and not convert from a colour profile, so this shot has not been post-processed.In my camera bag

Always meter off the brightest part of the image and focus on the eye closest to the lens axis. Also there might be a need to block any light that could leak onto the background, so card or any other material would need to be used to direct the light at the model and no where else. I also follow the rule of thirds by positioning the model so her eyes are just off center and place the model to the right or left of frame for a correctly balanced composition.
