The light setup is very simple - this is single light setup. This is actually 150WS studio flash equipped with 60x40 softbox.Equipment
I have used Canon 5D M2 with 85 1.2 lens, ND filter, and single studio flash.Inspiration
I recall this was a time when I have just purchase my 85 1.2 lens, and "played" with extremely low DOF. I have asked my little son to be a model and just did a series of shots, this one I have especially liked.Editing

There are couple of challenges when shooting with very low DOF. First - sharpness, it's not easy to get a focus in desired point. You need to make a lot of frames to get desired result. Second - using fast aperture (I have used 1.2) with regular studio flashes. Since you cannot use shutter faster than 1/250, you need to use ND filter, and it makes focusing even more challenging. Better (and more expensive) solution is using HSS studio flashes.
