This image was shot in my dining room with a Nikon D7000, and using one continuous light diffused by an umbrella. The pose the model (Katie Nicole) is in was achieved by combining three separate shots:
1. The main shot where her leg is extended in front. What you can't see is that she was being supported by my assistant (Joe Wozniak) so that she would not fall over.
2. A shot where she extended her back leg, and parts of her back from this one were used to cover up Joe's hands on the main image.
3. Finally I had her toss her hair in a third shot.
The water on the floor is from a photo I took of Pymatuning Reservoir. Multiple stone textures were used on the wall to achieve an aged look, and add the crack. Inside the crack I added a photo of an aquarium. To finish up the waterfall, splash, ripples, and the edges of the crack were hand painted using my Wacom tablet.
