Studio - Nikons: D300 and D2x with a D80 and D70s as backups. SB910, SB800 x 2 and SB600. Nikon 35mm f1.8, 50mm f1.8, 17-55 f2.8 and 70-200 f2.8. Tokina 11-16 f2.8. Travel - Fujifilm X100S with 23mm f2 and XT-1 with 18-55 f2.8 - 4. Yongnuo triggers, reflector, collapsible softobx and Phottix umbrellas, gaffer tape, chargers and batteries. It is a very easy shot to get. Single light camera left at 45 degrees, shoot through umbrella. I put baking powder inside the book and asked her to blow. Getting the facial expression right was the most difficult part as I wanted her to look like she is in awe but what she sees in the book and not pulling a blowing face. Took 12 frames, this was the best one. Try and keep your lighting as simple as possible.
