Well, the normal way I'd do this is set the camera on a tripod, take a frame with just the background and another with the model (keeping the focus locked). In this instance I didn't have my tripod, so I had to try and just hold really still, but that's the basic idea :) Then you can blend the two images together selectively in Photoshop, choosing how much of the model you want to see and where.
