Two seperate shots for the beer glass / hand and the rest. Cut out both and added a background made in PS using a radial gradient and smoke added in post.
Edited in Lightroom 4 and Photoshop CS5.

Shot with Canon 5D mark ii with Canon 17-40L @ 17mm.

Strobist:
24" softbox slightly camera left, directly front / above with Yongnuo YN560 @ 1/4 power.
40" umbrella directly behind subject with Yongnuo YN560 @ 1/2 power.
